Steps to Join Our Program

Submit an Inquiry (Agencies / Social Workers/ Case Managers)
Complete the Resident Qualification Sheet, Program Application Form, and upload required supporting documents.

Initial Assessment (Agencies / Social Workers/ Case Managers)
Our team will conduct an initial assessment to determine program suitability and discuss the applicant's needs and goals.

Submit the Application & Non-Refundable Application Fee (Future Residents)
Complete the Program Application Form. Submit the form, $75 non-refundable application fee, and supporting documentation (see below). We review all applications, consider the applicant's circumstances, support needs, and determine program fit. Please upload all required supporting documentation. Examples may include: Government-issued photo identification Social Security card Income verification Benefit award letters Referral documentation Emergency contact information Veteran documentation (if applicable)

Program Agreement
Upon acceptance, we develop an individualized support plan and complete the necessary program agreements.

Move-In
We coordinate a smooth transition into the program, working with the resident, family, and any referring agencies.
